The Central Mass Regional 911 District’s Document Portal provides convenient access to a wide range of public safety, administrative, and community forms and resources. Available documents include LTC/FID applications and change of address forms, the Massachusetts Firearms Transaction Portal, motor vehicle crash report forms, lost registration plate notices, and applications for disabled parking placards or plates. Residents can also find the 911 Disability Indicator Form, CORI personal request forms, and forms for residential house watches, site files (both residential and commercial), and citizen complaints or compliments. Additionally, the portal includes access to sex offender request forms (from the Police Department), department budgets, annual reports, board meeting minutes and agendas, Standard Operating Procedures (SOPs), and monthly reports from the Director.
